This data is aggregated and is not used to identify you personally. Where session analytics are used, form fields and signatures are masked, so information you type is not recorded.

Health information

Any health information you share with us during an appointment, intake form, or in-clinic conversation is protected under HIPAA and handled according to our Notice of Privacy Practices, which is provided to all patients at their first visit. This website does not collect or store clinical health records — those are maintained in our practice management system.
